Christian Medical & Dental Associations (“CMDA” or “We”) respect your privacy and are committed to protecting it through our compliance with this policy. This policy describes the types of information we may collect from you or that you may provide when you visit CMDA’s online properties, including websites, social networking platforms, blogs, mobile applications and/or through direct interaction with CMDA and our practices for collecting, using, maintaining, protecting and disclosing that information.

This policy applies to information we collect:

  • On this Website. (Collectively “the Website”).
  • In e-mail, text and other electronic messages between you, CMDA and its third party affiliates.
  • Through mobile and desktop applications you download.
  • When you interact with our advertising and applications on associated CMDA websites and with associated services, if those applications or advertising include links to this policy.
  • In other direct communications with CMDA, including but not limited to donating, membership, or event attendance or participation in a conference, meeting or mission trip.

It does not apply to information collected by:

  • Any third party, including through any application or content (including advertising) that may link to or be accessible from or on the CMDA’s website, mobile application or social networking sites.

Please read this policy carefully to understand our policies and practices regarding your information and how we will treat it. If you do not agree with our policies and practices, your choice is not to use our Website. By accessing or using the Website or interacting with CMDA through the other modes listed above, you agree to this Privacy Policy. This policy may change from time to time. Your continued use of the Website after we make changes is deemed to be acceptance of those changes, so please check the policy periodically for updates.

Children Under the Age of 18

The Website is not intended for children under 13 years of age. No one under age 13 may provide any personal information to or on the Website or through other means. We do not knowingly collect personal information from children under 13. If we learn we have collected or received personal information from a child under 13 without verification of parental consent, we will delete that information. If you believe we might have any information from or about a child under 13, please contact us at webmaster@CMDA.org.

We request that minors who are less than 18 years old, receive parental permission prior to sending any information about themselves to anyone over the Internet, and we encourage parents to teach their children about safe Internet use practices.

Information We Collect about You and How We Collect It:

We can collect several types of information from and about users of the Website, including information:

  • By which you may be personally identified, such as name, postal address, e-mail address, telephone number, and any other identifier by which you may be contacted online or offline (“personal information“);
  • That is about you but individually does not identify you; and/or
  • About your internet connection, the equipment you use to access our website, social networking sites or mobile applications and usage details.

This includes information provided at the time of creating an account to use with portions of the Website, creating an account to purchase materials or services, becoming a member of CMDA, when commenting on content, attending an event or requesting further services.   • Your responses to surveys that we might ask you to complete for research purposes.

Your User Contributions are posted on and transmitted to others at your own risk. We cannot control the actions of other users with whom you may choose to share your User Contributions. Therefore, we cannot and do not guarantee that your User Contributions will not be viewed by unauthorized persons.

  • Details of your activity, including traffic data, location data, logs and other communication data and the resources that you access and use.

We also may use these technologies to collect information about your online activities over time and across third-party websites or other online services behavioral tracking. This site does not respond to “Do Not Track” web browser signals.

The information we collect automatically is statistical data and may include personal information, but we may maintain it or associate it with personal information we collect in other ways or receive from third parties. It helps us to improve the Website and to deliver a better and more personalized service, including by enabling us to:

We do not collect personal Information automatically, but we may tie this information to personal information about you that we collect from other sources or you provide to us.

Third-Party Use of Cookies and Other Tracking Technologies.

Some content or applications are served by third-parties, including servers, content providers and application providers. These third parties may use cookies alone or in conjunction with web beacons or other tracking technologies to collect information about you when you use our website. We do not control these third parties’ tracking technologies or how they may be used.

For example, if you give us an e-mail address to use in a particular feature of our website, such as a “share this email with a friend,” for example, we will transmit the contents of that e-mail and your e-mail address to the recipients, as you would expect.
  • For any other purpose disclosed by us when you provide the information.
  • To present our website, social networking platforms, mobile applications and their contents to you.
  • With your consent.

We may also disclose your personal information:

  • To comply with any court order, law or legal process, including to respond to any government or regulatory request.
  • To enforce or apply our Terms of Use or other agreements, including for billing and collection purposes.
  • If we believe disclosure is necessary or appropriate to protect the rights, property, or safety of CMDAs, our users, donors or others. This includes exchanging information with other companies and organizations for the purposes of fraud protection and credit risk reduction.

Choices About How We Use and Disclose Your Information

We strive to provide you with choices regarding the personal information you provide to us. We have created mechanisms to provide you with the following control over your information:

  • Tracking Technologies and Advertising. You can set your browser to refuse all or some browser cookies, or to alert you when cookies are being sent. To learn how you can manage your Flash cookie settings, visit the Flash player settings page on Adobe’s website. If you disable or refuse cookies, please note that some parts of this site may then be inaccessible or not function properly.
  • Promotional Offers from the Ministry. If you do not wish to have your e-mail address/contact information used by the Ministry to promote our own or third parties’ products or services, you can opt-out through some electronic means on the form on which we collect your data. If we have sent you a promotional e-mail, you may click “unsubscribe” or “email preferences” and follow the directions for opting in or out of further correspondence.

Accessing and Correcting Your Information.

You can review and change your personal information, and will be able to adjust your privacy settings (coming soon) by logging into your account profile page. You may also send us an e-mail at webmaster@CMDA.org to request access to, correct or delete any personal information that you have provided to us. We cannot delete your personal information except by also deleting your user account. We may not accommodate a request to change information if we believe the change would violate any law or legal requirement or cause the information to be incorrect. If you delete your User Contributions, copies of your User Contributions may remain viewable in cached and archived pages, or might have been copied or stored by other Website users. Proper access and use of User Contributions is governed by our Terms of Use.

Your California Privacy Rights.

California Civil Code Section § 1798.83 permits users of our website that are California residents to request certain information regarding our disclosure of personal information to third parties for their direct marketing purposes. That information is contained in this document. To make such a request, please send an e-mail to webmaster@CMDA.org.

Data Security.

We have implemented measures designed to secure your personal information from accidental loss and from unauthorized access, use, alteration and disclosure. All information you provide to us is stored on our secure servers behind firewalls. Any payment transactions will be encrypted using SSL technology.

The safety and security of your information also depends on you. Where we have given you (or where you have chosen) a password for access to certain parts of the Website, you are responsible for keeping this password confidential. We ask you not to share your password with anyone. We urge you to be careful about giving out information in public areas like message boards or social media posts because the information you share in some public areas may be viewed by any user of the platform.

Unfortunately, even though CMDA will take every possible security measure to protect your information as outlined above, the transmission of information via the internet is not completely secure. Although we do our best to protect your personal information, we cannot guarantee the security of your personal information transmitted to the Website. Any transmission of personal information is at your own risk. We are not responsible for circumvention of any privacy settings or security measures contained on the Website.

Changes to Our Privacy Policy.

It is our policy to post any changes we make to our privacy policy on this page with a notice that the privacy policy has been updated on the home page of CMDA’s website. If we make material changes to how we treat our users’ personal information, we will notify you through a notice on the website home page. The date the privacy policy was last revised is identified at the top of the page. You are responsible for ensuring we have an up-to-date active and deliverable e-mail address for you, and for periodically visiting the Website and this privacy policy to check for any changes.

Awareness.  CMDA provides this Donor Privacy Policy to make you aware of our privacy policy, and to inform you of the way your information is used. We also provide you with the opportunity to remove your name from our mailing list, if you desire to do so.

Information collected.  Here are the types of donor information that we collect and maintain:

  • Contact information:  name, organization, complete address, phone number, email address
  • Payment information: credit card number and expiration date, and billing information
  • Shipping information: name, organization, complete address
  • Information concerning how you heard about CMDA
  • Information you wish to share: questions, comments, suggestions
  • Your request to receive periodic updates; e.g., upon individual request, we will send periodic communications related to specific fundraising appeals, prayer concerns, and newsletters, etc.

How information is used.  CMDA uses your information to understand your needs and provide you with better service. Specifically, we use your information to help you complete a transaction, to communicate back to you, and to update you on ministry happenings. Credit card numbers are used only for donation or payment processing and are not retained for other purposes. We use the comments you offer to provide you with information requested, and we consider carefully each recommendation as to how we might improve communication.

No sharing of personal information.  CMDA will not sell, rent, or lease your personal information to other ministries. We assure you that the identity of all our donors will be kept confidential. Use of donor information will be limited to the internal purposes of CMDA and only to further the ministry activities and purposes of CMDA.

Removing your name from our communications.  It is not our desire to send unwanted communications to our donors. Please contact us if you wish to be removed from our communications list.

By accessing these Services, you are agreeing to be bound by these Terms, all applicable laws and regulations, and agree that you are responsible for compliance with any applicable local laws.   5. Your User Submissions do not infringe the copyright, trademark, patent, trade secret, or other intellectual property rights, privacy rights, or any other legal or moral rights of any third party;
  6. You voluntarily agree to waive all “moral rights” that you may have in your User Submission;
  7. Any information contained in your User Submission is not known by you to be false, inaccurate, or misleading;
  8. Your User Submission does not violate any law (including, but not limited to, those governing export control, consumer protection, unfair competition, anti-discrimination, or false advertising);
  9. Your User Submission is not, and may not reasonably be considered to be, defamatory, libelous, hateful, racially, ethnically, religiously, or otherwise biased or offensive, unlawfully threatening, or unlawfully harassing to any individual, partnership, or corporation, vulgar, pornographic, obscene, or invasive of another’s privacy;

  15. You are solely responsible for the content of all information you contribute, link to, or otherwise upload to CMDA Websites; and
  16. You release and indemnify CMDA and sponsors of CMDA Websites from any claims and/or liability related to your use of those Websites.

Content Contributors

  1. CMDA publishes user-contributed content in the form of blog posts and comments. By providing blog posts either via RSS feed or directly on the site, comments or other content to the site, contributors agree to the following:
  2. Contributors may cease contributing new content at any time by emailing marketing@cmda.org
  3. CMDA reserves the right to not publish contributed content at CMDA’s discretion.
  4. CMDA will attribute content to the author.
  5. CMDA reserves the right to syndicate all contributed content in summary form via RSS to third parties.
  6. All contributed content may contain the author’s byline, a very brief biography of the author, and, in the case of material provided to us from the author’s own blog or by other submitted means, a link back to the original post in the footer of the full post.
  7. Contributing authors are solely responsible for the originality and accuracy of their submissions.
  8. CMDA has a zero-tolerance policy for plagiarism. CMDA recognizes that blogging is a heavily referential medium and that bloggers frequently cite and quote from each other’s work. Such citations might be protected under the legal doctrine of fair use. However, CMDA will remove any post that crosses the line from fair use to plagiarism, at CMDA’s sole discretion. Authors who knowingly submit plagiarized work may be banned from CMDA communities.
  9. CMDA reserves the right to remove any sharing links or advertising accompanying contributed posts.
  10. CMDA reserves the right to set fonts and styles, adjust image alignments, and otherwise alter post layout to insure that posts comply with CMDA’s global site styling.
  11. From time to time, CMDA may need to display summary versions of contributed content. In such cases, CMDA may edit posts in summary form. Such summaries will contain a link to the full and unaltered post. In such circumstances, CMDA will strive to preserve the original meaning and intent of the content.
  12. CMDA reserves the right to edit post headlines for clarity or search engine optimization (keyword inclusion).
  13. CMDA reserves the right to make corrections for spelling and grammar.
  14. CMDA reserves the right to reproduce blogger bios and accompanying pictures in CMDA’s marketing and promotional materials.
  15. CMDA reserves the right, at their discretion, to remove and block users who submit any content containing profanity, pornographic content, or personal/organizational attacking speech/language of any kind, at any time.
  16. CMDA reserves the right to refuse to post any comment on their website(s) for any reason, at any time.


CMDA’s social communities are intended to enable lively but civil interaction. Use of language that is abusive, off-topic, uses foul language, or includes ad hominem attacks will not be tolerated. CMDA reserves the right to determine what constitutes inappropriate behavior and to bar offenders at its sole discretion. CMDA reserves the right to block or remove postings or communications at any time at CMDA’s sole discretion.

Some links on CMDA Websites may link to third-party sites. Such links are provided solely as a convenience to you. If you use these links, you will leave the CMDA Websites. CMDA and any other product or service name or slogan or logo contained in the CMDA Websites, mobile app or associated brands under the CMDA umbrella of ministries are trademarks of CMDA or its departments, chapters, associations, suppliers or licensors and may not be copied, imitated, or used, in whole or in part, without the prior written permission of CMDA or the applicable trademark holder. Ownership of all such trademarks and the goodwill associated therewith remains with CMDA or the applicable trademark holder. You may not use any metatags or any other “hidden text” utilizing any name, trademark, or product or service name of CMDA without CMDA’s prior written permission. In addition, the look and feel of the Site (including all page headers, custom graphics, button icons, and scripts) is the service mark, trademark, and/or trade dress of CMDA and may not be copied, imitated, or used (in whole or in part) without CMDA’s prior written permission. Procedure for making claims of infringement

CMDA respects the intellectual property rights of others. Accordingly, CMDA has a policy of removing User Submissions that violate copyright law, suspending access to the CMDA Websites (or any portion thereof) to any user who uses the CMDA Websites in violation of copyright law and/or terminating in appropriate circumstances the account of any user who uses the CMDA Websites in violation of copyright law. International and export issues

CMDA makes no representation that the CMDA Services are appropriate or available for use outside the United States and access to the CMDA Services from territories where its contents are illegal or restricted is prohibited. If you choose to access the CMDA Services from outside the United States, you do so on your own initiative and are responsible for compliance with applicable Laws.


You agree to indemnify, defend, and hold harmless the CMDA Parties from and against any and all claims, liabilities, damages, losses, costs, expenses, or fees (including reasonable attorneys’ fees) that such parties may incur as a result of or arising from your (or anyone using your account’s) violation of these Terms of Use. CMDA reserves the right to assume the exclusive defense and control of any matter otherwise subject to indemnification by you and, in such case, you agree to cooperate with CMDA’s defense of such claim.

Electronic communications

When you visit the Site or send e-mails to CMDA, you are communicating with CMDA electronically. CMDA may respond to you by e-mail or by posting notices on the Site. You agree that all such notices, disclosures, and other communications that CMDA provides to you electronically satisfy any legal requirement that such communications be in writing.


These Terms of Use constitute the entire and exclusive and final statement of the agreement between you and CMDA with respect to the subject matter hereof, superseding any prior agreements or negotiations between you and CMDA with respect to such subject matter. The section titles in these Terms of Use are for convenience only and have no legal or contractual effect. These Terms of Use shall remain in full force and effect notwithstanding any termination of your use of the CMDA Websites. These Terms of Use will be interpreted without application of any strict construction in favor of or against you or CMDA. These Terms of Use, and any rights and licenses granted hereunder, may not be transferred or assigned by you, but may be assigned by CMDA without restriction.

Modifications to these Terms of Use

CMDA may, in its sole and absolute discretion, change these Terms of Use from time to time. CMDA will post notice of such changes on the applicable webpage. If you object to any such changes, your sole recourse shall be to cease using the CMDA Websites. Continued use of the CMDA Websites following notice of any such changes shall indicate your acknowledgement of such changes and agreement to be bound by the terms and conditions of such changes. We may in the future need to restrict access to parts (or all) of our Services and reserve full rights to do so. The materials within these services are provided "as is" without any express or implied warranty of any kind including warranties of merchantability, noninfringement of intellectual property, or fitness for any particular purpose. In no event shall cmda or its agents or officers be liable for any damages whatsoever (including, without limitation, damages for loss of profits, business interruption, loss of information, injury or death) arising out of the use of or inability to use the materials, even if CMDA has been advised of the possibility of such loss or damages.


If you wish to purchase any membership, product or service, or give a donation made available through any of CMDA’s Services, you may be asked to supply certain information relevant to your Purchase including, without limitation; your full name, your permanent email address, your credit card or PayPal information, your age, your professional service level (i.e.: first year student, resident, practitioner, etc.) your degree, etc.


We may terminate your access to the Services, without cause or notice, which may result in the forfeiture and destruction of all information associated with you. All provisions of this Agreement that by their nature should survive termination shall survive termination, including, without limitation, ownership provisions, warranty disclaimers, indemnity, and limitations of liability.

Protected Content

Our Services allow you to post, link, store, share and otherwise make available certain information, text, graphics, videos, or other material ("Content"), which has been opened to the public or to specific groups for the purposes of promoting CMDA and it’s Services. The Christian Doctor Search and CMDA Colleague Search use our extensive member database to return names and contact information of physicians and dentists matching your search criteria. CMDA strongly enforces the confidentiality of our members' public and personal information. By performing a search on our member database with one or both of these tools, you, the user, understand and agree to abide by the following terms and conditions:

  • The Christian Doctor Search is provided solely for the purpose of helping individuals locate a Christian doctor in their area.
  • The CMDA Colleague Search is to be used only by CMDA members for either 1) patient referral: self-referral or doctor-referral, or 2) member networking for CMDA ministry purposes.
  • The information provided through either search tool may not be copied, redistributed, or otherwise used in any way that will advance the financial interests of the user.
  • The Christian Medical & Dental Associations make no warranty, expressed or implied, nor accept any liability for any of the information in our member database, or the use of that information.
  • Misuse of the Christian Doctor Search and/or CMDA Colleague Search tools, as deemed by the terms listed above, may result in the suspension of CMDA membership and/or legal prosecution against the user.

Satisfaction is guaranteed for the items purchased through the Life & Health Resources bookstore of CMDA. Items may be returned, unused, within 90 days for a refund. CMDA Logo Graphic

Under no circumstances can the CMDA logo be used for marketing purposes.

If you use the CMDA logo for any reason on your Web site, printed materials, video or custom apparel, you agree to use only approved logo graphics and will not tamper with the image or change it in any way. The CMDA, CDA and CMA logos cannot be used for personal or business marketing purposes.

Please note the two fonts. The top font is Schoolbook. This font is NOT to change. The bottom font is Textile. This is for tag lines and the font is NOT to change. These fonts define our image and provide the consistency necessary for adaptability. The red is PMS 1797, CMYK Process Color 94% magenta, 94% yellow, 6% black (no white). This red may be used or you may use black. The full name should be used. No acronyms are acceptable in the logo usage.

In addition, the trademark symbol is to accompany all logo usages in the position as shown.

You may reduce or enlarge the entire logo proportionately. This can be done in Microsoft Office Word by clicking on the logo and then dragging a corner point while holding down the shift key this allows the size to change without distortion.

In Regard to Credit Card Transactions Made Through CMDA

I understand that this authorization will remain in effect until I cancel it in writing, and I agree to notify Christian Medical & Dental Associations in writing of any changes in my account information or termination of this authorization at least 15 days prior to the next transaction date. If the noted payment dates fall on a weekend or holiday, I understand that the payments may be executed on the next business day. For ACH debits to my checking/savings account, I understand that because these are electronic transactions, these funds may be withdrawn from my account as soon as the above noted periodic transaction dates. I acknowledge that the origination of ACH transactions to my account must comply with the provisions of U.S. law. I certify that I am an authorized user of this credit card/bank account and will not dispute these scheduled transactions with my bank or credit card company; so long as the transactions correspond to the terms indicated in this authorization form.

I hereby authorize Christian Medical & Dental Associations to obtain or determine updated or replacement expiration dates for your credit card in the event that the credit card you provided us expires. We reserve the right to charge any renewal card issued to you to the same extent as the expired card.
